09/13/2018; 09/28/2018; 10/24/2018 Unannounced complaint inspections were conducted on the above dates in response to complaints reported to the licensing office concerning ratios and supervision of children in care. Based on interviews, observation and the review of documentation, there IS evidence to support the allegations. A violation notice WILL BE issued. Thank you for your cooperation. Description: Based on a review of staff files, the facility failed to insure that all staff submit documentation of a negative TB screening no later than 21 days after employment and shall have been completed 12 months prior to employment. Evidence: 1. 09/28/18 There are no TB screening results on file for staff #8 (hired 8-13-18) or 9 (hired 8-8-18). 10/24/18 There are no TB screening results on file for staff #3 (hired 9-17-18) 2. 10/24/18 The TB screening results on file for staff #2 (hired 9-10-18) are dated 01-23-17. The TB screening results on file for staff #6 (hired 9-10-18) are dated 01-27-17.

Plan of Correction: The director will insure that all staff have TB screening results on file before they begin work.

Description: Based on the review of 12 staff files, the facility failed to obtain required references and conduct orientation for all staff. Evidence: 1. There are no reference checks or proof of orientation on file for Staff #4, 8, 9,10,11,12.

Plan of Correction: The director will obtain references for all new staff and conduct orientation for all new staff.

Description: Based on observation, the facility failed to maintain staff to child ratios. Evidence 1. On 9/28/2018 at 11:25am the licensing inspector observed that there were three staff on duty with 15 infants.

Plan of Correction: The director will insure that staff to child ratios are followed during operating hours.

Complaint related: Yes
Description: Based on observation, the facility failed to maintain staff to child ratios. Evidence 1. On 10/24/2018 at 10:10am the licensing inspector observed that there were two staff on duty with 17 two and three-year-olds. And in the twos' class, there were two staff on duty with 15 children, one of whom is not yet two years of age.

Plan of Correction: The director will insure that staff to child ratios are followed during operating hours.

Description: Based on a review of staff files, the facility failed to insure that staff members complete a sworn statement prior to employment or volunteering. Evidence: 1. 09/28/18 There are no sworn statements on file for staff #6, 7 or 8. 2. Staff #9 began work on 8-8-18. The sworn statement was signed 9-25-18.

Description: Based on a review of staff files, the facility failed to insure that staff members undergo a central registry background check within 30 days of employment. Evidence: 1. 09/28/18 There are no central registry clearances on file for staff #8 (hired 8-13-18), staff 9 (hired 8-8-18).

Plan of Correction: The director will insure that new staff complete all required licensing paperwork upon hire before beginning work.

Evidence: 1. 09/28/18 There are no fingerprint background checks on file for staff #2 (hired 9/10/18), 3 (hired 9-17-18), 5 (hired 1-16-17), 6(hired 9-10-18), 7 (volunteer) 8 (hired 8-13-18). 2. Staff #4 began work on 9-24-18. The fingerprint background check is dated 9-26-18. 3. Staff #9 began work on 8-8-18. The fingerprint background check is dated 9-7-18. 4. 10/24/18 There is no fingerprint background check on file for staff #10. (hire date 10-18-18)

Plan of Correction: The director will insure that all new staff have completed fingerprint background checks upon hire before beginning work.
