The licensing inspector conducted an unannounced complaint inspection in response to a complaint that was received by the licensing department on October 11, 2018 regarding allegations of forbidden actions, behavioral guidance, supervision, diapering, sanitizing, and hand washing. The inspection was completed on October 25, 2018. Interviews with staff were completed, observations were made in the center, and documentation was reviewed. The preponderance of the evidence gathered during the investigation does not support the allegation; therefore, the complaint is determined to be not valid. Though not a part of the original complaint, there were violations found during the inspection unrelated to the allegations. See the violation notice for more details. Description: Based on a review of records, the center failed to keep a written record of children in attendance each day. EVIDENCE: 1. The licensing inspector observed that there were 11 children present in the infant/toddler classroom and 7 children were listed on the attendance sheet. 2. The licensing inspector observed that there were 14 children present in the 3 year old classroom and 8 children were listed on the attendance sheet. 3. The licensing inspector observed that there were 10 children present in the 4 year old classroom and no children were listed on the attendance sheet.

Plan of Correction: Administration will consult with staff and ensure that attendance sheets are accurate throughout the day.

Description: Based on inspection of the facility, the center did not ensure all hazardous substances were kept in a locked place. Evidence: 1. The cabinet in the kitchen contained bleach, sanitizers, and cleaners and the unit was not locked. There was a container of bleach water on the refrigerator. The kitchen door was open and unlocked. 2. The licensing inspector observed a container of disinfectant wipes on a table in the 3 year old classroom. Hazardous substances shall be kept in a locked place using a safe locking method that prevents access by children.

Plan of Correction: Administration will ensure that all hazardous materials are stored in a locked area that prevents access by children.
