A complaint investigation was initiated on October 10, 2018 and ended on October 17, 2018 regarding a staff member's forbidden action against a child. After reviewing the center's video-camera footage of the incident, interviewing the Director, and review of 1staff record, there was preponderance of evidence to support the allegation regarding the rough handling of a child and therefore, the complaint is valid. The one complaint related violation and two non- complaint related violations are identified on the Violation Notice. Standard #: 22VAC40-185-410-1 Complaint related: Yes Description: Based on review of video-camera footage and interview with Staff, rough handing of a child was observed. Evidence: On 10/10/18, the center's video footage was observed for an incident that occurred on 9/14/18. Staff#1 was observed forcibly grabbing Child#1 by their arm, who had taken off their shoes, and roughly sitting the child on the floor to put their shoes on. Plan of Correction: Staff was put on Administrative Leave on 10/10/18, and ultimately terminated. Director stated she would be conducting a staff training on positive guidance techniques soon.
Standard #: 22VAC40-185-500-A Complaint related: No Description: Based on review of center's video-camera footage, Staff and children's hands were not washed with soap and water after diapering. Evidence: 1. On 10/10/18 while reviewing video footage of 9/14/18, Staff#1 was observed changing 2 children's diapers and did not wash either of the children's hands after diapering. 2. Staff#1 was observed on video footage changing 2 children's diapers. Staff#1 did not wash their own hands after each diaper change. Plan of Correction: I will remind staff of the diapering procedure.
Standard #: 22VAC40-185-500-B Complaint related: No Description: Based on review of center's video-camera, the diapering surface was not cleaned with soap and at least room temperature water and sanitized after each use. Evidence: Staff#1 was observed changing 2 children's diapers without cleaning and sanitizing the diapering surface between each diaper change. Plan of Correction: I will review diapering procedure with the staff
