A complaint investigation was initiated on October 10, 2018 and ended on October 17, 2018 regarding a staff member's forbidden actions against two children. After reviewing the center's video-camera footage of the incident, interviewing the Director, and review of 1staff record, there was preponderance of evidence to support the allegation regarding the rough handling of children and therefore the complaint is valid. Standard #: 22VAC40-185-410-1 Complaint related: Yes Description: Based on review of center's video-camera recording and interview with Staff, a forbidden action of roughly handling 2 children was observed. Evidence: On 10/10/18, video footage from 10/09/18 was reviewed. At approximately 4:00 pm of the video tape, Staff#1 was observed simultaneously grabbing Child#1 and Child#2 by their arms and forcibly removing them from standing on a chair. Child#1 and Child #2 were then roughly sat on the floor. Plan of Correction: Staff#1 was placed on Administrative Leave and ultimately terminated. Director stated she would conduct a staff meeting on positive guidance techniques.
