Comments:
Thank you for your assistance during this unannounced monitoring inspection. This on-site visit was conducted on October 29, 2018 from 11:00 a.m. through 2:30 p.m. There were eight-four children in the care of seventeen staff. Eight children's and eight staff records were reviewed and updated. No medication is being administered at this time. Areas of non-compliance may be found on the violation notice. Areas observed were free choice activity, infant feeding, hand washing/diapering procedures, transition and nap. Interactions between the children and staff were observed as appropriate and the children seem comfortable with the daily routine and schedule. Standard #: 22VAC40-185-160-C
Description: Based on record review, the center failed to ensure one staff member had obtained an updated tuberculosis screening. Evidence: 1. Eight staff records were reviewed. 2. Staff #2 did not have documentation of obtaining an updated tuberculosis screening. Last documented updated was dated 09/15/2016. 3. The director confirmed there was not any documentation of an updated tuberculosis screening.

Plan of Correction: Staff #2 will obtain an updated tuberculosis screening immediately.

Standard #: 22VAC40-185-450-A
Description: Based on record review, the center failed to ensure that two children had bottom sheets for their cots when napping. 1. Classroom #4 had two children napping on cots that did not have bottom sheets. 2. Staff confirmed their were not any bottom sheets for the two children.

Plan of Correction: We have extra sheets. Staff will be reminded to ask administration for extra sheets.
