Reminder to staff: After every diaper, the changing table shall be cleaned with soap and water and then sprayed with bleach water solution. Allow the bleach water to air dry for a period of at least two minutes and then dry to proceed with next diaper.
An unannounced renewal inspection was conducted on October 29, 2018 with center director to determine compliance with the licensing standards. There were four children present, ages 19 months, 2 years, 4 years, and 4 years, with two staff supervising. The children finished lunch and went down for rest time. Lunch served today: fish sticks, tater tots, applesauce, bread, and milk. There are no children receiving medication at the center. Four children's files and three staff files were reviewed - please see Supplemental Summary for missing items. The last health inspection was conducted 8/8/2018. The last fire inspection was conducted 7/20/2017 - please follow up with fire marshal to schedule an inspection. The last shelter in place drill was conducted 9/13/2018. Standard #: 22VAC40-185-130-A Description: Based on review of four children's records, the facility failed to have documentation of an immunization record for each child prior to the child's the first day of attendance. Evidence: There was no documentation on file of an immunization record for Child A (date of enrollment 9/10/2018). Plan of Correction: Provider will request the information from the parent.
Standard #: 22VAC40-185-140-A Description: Based on a review of four children's records, the facility failed to have documentation of a physical exam for each child within one month of the first day of attendance. Evidence: There was no documentation on file of a physical exam for Child A (date of enrollment 9/10/2018). Plan of Correction: Center director will request the information from the parent.
Standard #: 22VAC40-185-90--A Description: Based on review of four children's records, the facility failed to have a written agreement between the parent and center by the first day of attendance including a signed statement that the center will notify the parent when the child becomes ill and that the parent will arrange to have the child picked up as soon as possible if so requested by the center and a signed statement that the parent will inform the center within 24 hours or the next business day after his child or any member of the immediate household has developed any reportable communicable disease. Evidence: there was no written statement signed by Child A's parent. Plan of Correction: Center director will request the parent sign this agreement for the file.
Standard #: 22VAC40-185-260-A Description: Based on review of center inspections, the facility failed to have an annual fire inspection from the appropriate fire official as required. Evidence: the last documented fire inspection was conducted 7/20/2017. Plan of Correction: Center director has called the fire marshal's office several times requesting an inspection.
Standard #: 63.2-1809-A Description: Based on review of four children's records, the facility failed to have documentation of proof of identity within seven days of first attendance. Evidence: there was no proof of identity on file for Child A (date of enrollment 9/10/2018. Plan of Correction: Center director will request the information from the parent.
