22VAC40-191 Background Checks (22VAC40-191)
An unannounced monitoring inspection was conducted today from 11:00am-3:30pm. There were 39 children (infants-11yrs.) directly supervised by 7 staff. The physical plant, 5 staff records, 5 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, drawing, tracing patterns, learning about objects that sink and objects that float, playing with magnetic tiles and eating snack. Diapering and hand-washing procedures were also observed. There was an abundant supply of materials for the children. The center was clean and organized. Areas of non-compliance are identified on the Violation Notice. Standard #: 22VAC40-185-130-A Description: Based on record review, it was determined that the center did not have immunization records for each child in care. Evidence: Child#2's first day of attendance was documented as 7/9/2018, the child's record did not contain documentation of the child's immunization record. Plan of Correction: Corrected during the inspection.
Standard #: 22VAC40-185-140-A Description: Based on record review, it was determined that the center did not have a physical examination for each child in care. Evidence: Child#2's file did not contain a physical immunization required within 30 days of attendance. The child's first day of attendance was documented as 7/9/2018. Plan of Correction: Corrected during the inspection.
Standard #: 22VAC40-185-280-B Description: Based on observation, it was determined that the center did not keep cleaning materials in a locked place using a safe locking method. Evidence: In the Pre-k classroom, a spray bottle of glass and multipurpose cleaner and a spray bottle of disinfectant were observed in an unlocked upper cabinet. Plan of Correction: We are going to retrain staff on safe storage of cleaning supplies.
Standard #: 22VAC40-185-510-C Description: Based on review of medication documentation and staff interview, it was determined that the center had an emergency prescription medication on-site with a medication authorization form signed by the parent only, the form was not signed by the child's physician as required for long term medications. Evidence: Child#1 has a long term emergency medication on-site with an authorization form that was signed by the child's parents but the authorization forms was not signed by the child's physician. Plan of Correction: We have requested the information during the inspection.
