22VAC40-191 Background Checks (22VAC40-191)
An unannounced monitoring inspection was conducted on October 24, 2018 from 2:30-6:00pm. Upon arrival to the facility, there were 21 children present with three staff members. The licensing inspector reviewed supervision, staff qualifications and trainings, nutrition, required posting, the physical plant and administration. The center is not administering medication at this time. The children were arriving from school, eating snack and waking up . Exit interview conducted with the Program Director.
Standard #: 22VAC40-185-160-C Description: Based on 1 of 5staff records reviewed, the licensee did not obtain documentation of a negative tuberculosis screening at least every two years. Evidence: During the inspection conducted on October 24, 2018, staff #3, the most recent T.B. test was 2008. Plan of Correction: The staff will have a T.B. test completed tomorrow.
Standard #: 22VAC40-185-70-A Description: Based on 2 of 5 records reviewed, the licensee did not ensure that each staff record was documented with all required information. Evidence: During the inspection conducted on October 24, 2018, the follow documents were missing: Staff #1 did not have references, emergency contact information, orientation documention, start date and title of position. Staff #2 did not have references. Plan of Correction: Staff #1 just starting as a volunteer . The volunteer packet is not complete . The program director will locate the references and will be put in her record.
Standard #: 22VAC40-185-340-A Description: Based on observation, the licensee did not ensure their care and protection and guidance. Evidence: During the inspection conducted on October 24, 2018, the infant activity table with bucket seats there was one child observed not secured in the seat and further observation showed straps were missing. The center director was not sure where the straps were located. Plan of Correction: No children will be put in the infant activity table seats.
Standard #: 22VAC40-185-350-E-1 Description: Based on observation, the licensee did not ensure for children from birth to the age of 16 months: one staff member for every four children. Evidence: During the inspection conducted on October 24, 2018 at 2:25 pm- 2:55pm, there was one staff with eleven children from the age 7 months to 12 years of age. Plan of Correction: The center director will communicate clearer to the staff.
Standard #: 22VAC40-185-560-F Description: Based on observation and staff interview, the licensee did not ensure centers followed the most recent, age-appropriate nutritional requirements of a recognized authority such as the Child and Adult Care Food Program of the United States Department of Agriculture (USDA). Evidence : During the inspection conducted on October 24, 2018, the children in the 2 year old -4 year old classroom were served graham crackers and water. There were not two of four components for snack. Plan of Correction: The program director stated snack should of included apple juice 100%.
Standard #: 22VAC40-191-60-B Description: Based on record review the licensee did not ensure a completed sworn statement or affirmation before they are employed or provide volunteer service. Evidence: During the inspection conducted on October 24, 2018, the record for Staff #1 did not have a sworn statement or affirmation. Plan of Correction: Corrected while inspector present.
Standard #: 22VAC40-191-60-C-2 Description: Based on 1 of 6 records reviewed , the licensee did not ensure staff had a central registry finding within 30 days of employment or volunteer service. Evidence: During the inspection conducted on October 24, 2018, the record for Staff #2 (date of hire 7/18/2018) did not include documentation of central registry results. Plan of Correction: The program director thought the fingerprint covered Central registry.
