A Subsidy Health and Safety inspection was conducted on Monday, October 29, 2018 from approximately 9:56 am to 3:45 pm to review supplemental health and safety requirements. The children in care were observed by the licensing inspector in the various classroom settings. The census for today's inspection consisted of 174 children and 25 staff present. There were additional staff on site for administrative and support purposes. The program's theme for today was jersey day. The licensing inspectors observed the children in care completing a variety of activities to include engaging in active play with peers and age appropriate equipment, hand painting, working on the letter G, identifying orange things and painting each item, eating snack, singing songs in circle time, actively participation in music class, participating in the book fair, returning from a field trip, completing bathroom breaks, and transitioning for lunch time. The children's lunch for today consisted of chicken pot pie, oranges, and milk. The center's classrooms, bathrooms, and outdoor playground areas were inspected. During the inspection, the following information was reviewed by the licensing inspector: daily attendance, emergency supplies, injury reports, fire and shelter-in-place drills, liability insurance policy. There were six children?s records and six staff's record reviewed during this inspection. The required posting were inspected. Medication is being administered, medications, and written authorizations were reviewed for the center's compliance. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requiremen.ts. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. New subsidy requirements became effective on October 17, 2018. The facility is to review the new requirements and work with their assigned inspector to ensure future compliance. Standard #: VENDSUB-000-012-B Description: Based on review of staff and interview with staff, the center did not ensure all staff complete the required Virginia Preservice Training. Evidence: During the inspection conducted on October 29, 2018, in Staff#5's (DOH: 06/15/15) record the Virginia Preservice training was not found. Interview with staff revealed the staff had not completed required training at this time. Plan of Correction: The staff will complete training requirement by October 31, 2018
Standard #: VENDSUB-000-012-E Description: Based on review of staff records and interview with staff, the center did not ensure completion of required training within 30 days of the date of employment or 90 days from subsidy vendor. Evidence: During the inspection conducted on October 29, 2018, the following was not found in the staff record: 1. In Staff#3's record, (date of hire 6/29/15) the first aid completion training was not found. Interview with staff confirmed the staff first aid certificate was not found. 2. 1. In Staff#4's record, (date of hire 3/20/17) the first aid/CPR completion training was not found in the record. Interview with staff confirmed that trainings have not been completed at this time. Plan of Correction: 1. The center will obtain a copy of first aid certificate of completion. 2.The staff will complete FA/CPR by Monday, November 5, 2018
