- Technical Assistance:
Had an in depth conversation with center and regional management regarding medication authorization forms. LI will follow-up with center regarding adequacy of new authorization forms that the program has created.
Today an unannounced Monitoring Inspection took place between the hours of 9:20am and 1:40pm. At the start of the inspection there were a total of 83 children with 16 staff; all ratios were incompliance. A total of 5 children?s files and 6 staff files were reviewed. There were 15 medications on site today. The children were observed painting, participating in sensory activities, learning about insects getting caught in spider webs and playing outside. Standard #: 22VAC40-185-60-A Description: Based on record review, children's records did not contain all required information. Evidence: Child #1 file did not contain a phone number for their first emergency contact. Plan of Correction: Emergency contact was obtained and is on file.
Standard #: 22VAC40-185-280-D Description: Based on observation, cleaning and sanitizing materials were located above and near food and food equipment. Evidence: 1. In the teacher cabinet of the JKB classroom, there is a cleaning material stored above cake frosting, a bag of pretzels and sprinkles. On the same shelf as the food, there is also a large bottle of hand sanitizer. 2. In the cabinet above the sinks of the Prep A classroom, there is a bottle of fish tank cleaner being stored next to paper plates and plastic cups. Plan of Correction: During the inspection cleaning materials were removed from cabinet with food and put in separate locked cabinet.
Standard #: 22VAC40-185-510-A Description: Based on a review of medications and authorizations, medication is kept on-site without a parents written authorization to administer. Evidence: Child #8 currently has emergency medication kept on-site in case of an accidental ingestion of an allergen. The center has not obtained a written authorization to administer the medication. Plan of Correction: Appropriate paperwork was obtained and is on file.
Standard #: 22VAC40-185-510-N Description: Based on a review of medications and authorizations, medication was not returned to parents within 14 days after an authorization to administer had expired. Evidence: Child #7 currently has emergency medication kept on site. The parents completed a short term medication authorization which expired on 10/4/2018. During today's inspection the medication was still on-site. Plan of Correction: Appropriate paperwork was obtained and is on file.
Standard #: 22VAC40-185-520-A Description: Based on observation, over-the-counter skin products were kept beyond their expiration date. Evidence: Child #6's diaper cream expired 8/2018. Plan of Correction: Expired diaper creams were disposed of during the inspection. Retrained teachers on checking expiration dates.
Standard #: 22VAC40-185-570-E Description: Based on observation, infant bottles were not dated with today's date. Evidence: Child #1 had three bottles located in the refrigerator of the infant classroom. One bottle was dated 10/12, one bottle was dated 10/18, and one bottle's date was not legible. Plan of Correction: Retrained teachers on appropriate labeling of bottles to ensure that all bottles have full name and date on them.
