An unannounced monitoring inspection was conducted during the before care portion of the program on October 30, 2018. Upon arrival to the facility at 6:09am there were two staff members with no children present. Throughout the course of the inspection 11 children arrived. The children were observed arriving to the program from school and being signed in, talking freely amongst themselves and doing various tabletop activities to include play-doh, homework, reading books, playing board games and coloring. There are no medications present or being administered in the facility. Five children's records were reviewed during this inspection.

Standard #: 22VAC40-185-140-A Description: Based on record review, in one of five children's records reviewed, the licensee did not ensure that each child shall have a physical examination by or under the direction of a physician: Before the child's attendance or within one month after attendance. Evidence: During the inspection conducted on October 30, 2018, there was no documentation of a physical examination in the record for Child #4 (start date as indicated in the record 9/4/18). Plan of Correction: Site Director has contacted the parent who will btinh a copy at drop-off.
Standard #: 22VAC40-185-260-A Description: Based on review, the licensee did not provide to the licensing representative an annual fire inspection report from the appropriate fire official having jurisdiction. Evidence: During the inspection conducted on October 30, 2018, the most recent fire inspection report for the facility was more than one year old and was dated 4/6/17. Plan of Correction: The Area Manager is aware and is attempting to obtain a copy. The results are pending.
