Comments:
An unannounced renewal inspection was conducted beginning at 9:30 a.m. and ending at 12:45 p.m. with the director. There were 45 children in the care and supervision of 10 staff. Infants were observed sleeping, having story time with staff, playing on climber and being fed. Bottles were labeled and dated. Toddlers, two's, three's, four's and five's were observed participating in a center event Halloween magic show. Children were observed participating in the show and appeared to be enjoying the program. Classrooms and outdoor play areas were observed. Eight children's records and four staff records, including two new staff were reviewed. Staff training hours were reviewed. Six medications and authorizations were reviewed. Emergency drills are up to date. First aid and emergency supplies were reviewed. Standard #: 22VAC40-185-60-A
Description: Based on review of documentation, not all children's records were complete. Evidence: Child #1, enrolled 9/5/17, did not have two emergency contacts other than the parents listed in the record. Child #3, enrolled 7/9/18, did not have proof of age and identity recorded within 7 days of enrollment.

Plan of Correction: The parents/guardians were contacted during the inspection and the files will be updated by this evening.

Standard #: 22VAC40-185-270-A
Description: Based on observation, not all areas of the center were maintained in safe, operable condition. Evidence: On the school age playground, on the upper deck of the blue jungle gym there is a yellow composite panel that is loose and not secured to the equipment creating a fall and pinching hazard. On the school age playground, the red and yellow talking tube near the blue jungle gym has a bolt that has 5 threads exposed creating a scratching, impalement and entanglement hazard. On the toddler playground, the yellow and red composite climber has a stabilizer bar that is not secured at the top that creates a pinching hazard.

Plan of Correction: Maintenance was contacted during the inspection and will be here today to fix all the items. The school age playground will be off limits until items are fixed.
