Comments:
An unannounced monitoring inspection was conducted today from 11:15am-12:45pm. There were 86 children (infants-5yrs.) directly supervised by 17 staff. The physical plant, 5 staff records, 5 children?s records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, reading books, playing outside, and transitioning to lunch. No medications were available for review on the date of inspection. There was an abundant supply of materials for the children. The center was clean and organized. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-185-270-A
Description: Based on observation, not all areas within the center were maintained in a safe and operable condition. Evidence: 1. The rubber carpet stripping around the tiles surrounding the sink in Room H is peeling away from the floor creating a tripping hazard.

Plan of Correction: We are replacing all of the carpet in the center.

Standard #: 22VAC40-185-350-E-4
Description: Based on observation and staff statement, the ratio of 1 staff for every 10 children for children from three years to the age of eligibility to attend public school, five years was not maintained. Evidence: Classroom F was running at a ratio of 1 teacher for 20 children on the date of inspection for a period of no more than three minutes while a supervising staff member was on a restroom break.

Plan of Correction: We will remind staff to ask for coverage when they need to leave the room.

Standard #: 22VAC40-185-430-A
Description: Based on observation, not all materials were of an appropriate size for the child using it. 1. A child in room H was observed with a cylinder foam block in her mouth which was less than 1 1/12 inches in diamweter and could be considered a choking hazard. 2. Several toys are currently being stored in Room H as the result of a special school event. One of these yoys is not age appropriate: small building logs which are choking hazards fro children under 3.

Plan of Correction: We will remove all toys from this classroom that present a hazard.
