Discussion was held on the topics of current enrollment, current staffing, and Fire Marshall's report.

Comments:
The following inspection took place between the hours of 10 am through 1 pm. Today, I met with the Staff of the licensed preschool programs held on site at Claude Moore Recreation Center located in Sterling, Virginia. At the start of this unannounced monitoring inspection there were a total of 111 children with 15 classroom staff; good ratios in all 7 rooms. A total of 10 children?s files were reviewed. Staff files were reviewed on an earlier date through the PRCS main office in Ashburn. The children were engaged in the following: center activities within their classroom, making craft pumpkins, music, story time, learning about spiders, large motor activities on center playground, bathroom breaks, hand washing, snack, pick-up, and other organized activities. Standard #: 22VAC40-185-60-A
Description: Based on review, 1 of 10 children's files reviewed indicated that not all of the required information/documentation was contained within each child's record. Evidence - 1. On the date of inspection (10/24/2018), the emergency contact information for Child #3 was not available for review. 2. After a review of Child #3's file, the children's registration form for 2018 that would have contained this information was not on file.

Plan of Correction: The parent of Child #3 shall be contacted and the information shall be obtained and placed on file.

Description: Based on review, 1 of 10 children's file review indicated that the center did not request at least annually parent confirmation that the required information in the child's record is up to date. Evidence - 1. On the date of inspection (10/24/2018), the most recent children's registration form for Child #5 was dated - 08/17/2017. 2. This document was the most recent on file. The form for the current year was not available for review.

Plan of Correction: The current form shall be found or a new form shall be completed and placed on file.
