10/30/2018 Arrived 3:25 pm Departed 5:20pm An unannounced monitoring inspection was conducted today. Forty-seven children were in care with five staff and were eating a snack, doing homework and playing outdoors. Previous violations, five children's files and six staff files were reviewed. Standard #: 22VAC40-191-60-B Description: Based on the review of six staff files, the facility failed to insure that all staff members complete all background checks. Evidence: 1. There is no sworn statement on file for staff #6 who began volunteering 2-28-18 . Plan of Correction: The chief of staff will insure that staff #6 signs a sworn statement and that all future staff sign this form before beginning employment or volunteering.
Standard #: 22VAC40-191-60-C-2 Description: Based on the review of six staff files, the facility failed to insure that all staff members complete all background checks. Evidence: 1. There is no CPS Central Registry Check on file for staff #4 (hired 3-1-18) . It was completed and mailed 4/5/18. Plan of Correction: The form was returned by the Central Registry twice for further information. The chief of staff has called the Central Registry and left a message to inquire about the form.
