During the inspection, the licensing representative discussed: - resilience surfacing - playground area - updated CDC/subsidy regulations - age appropriate equipment
- Comments:

An unannounced monitoring inspection was conducted from 10:00am-1:00pm. At the time of entrance, 15 children were in care of 3 staff members present. Children were observed engaged in group activities, outdoor play time, handwashing and lunch. A selection of 3 staff and 5 children records and other documents were reviewed. Standard #: 22VAC40-185-60-A Description: Based on records, reviewed, 1 out of 5 children's records, did not include the required information. Evidence: The record for Child #2 was missing the addresses for 2 out of 2 required emergency contacts. Plan of Correction: We will have the parent add the information to the record.
Standard #: 22VAC40-185-270-A Description: Based on observations, areas inside and outside of the center were not maintained in clean, safe and operable condition. Evidence: 1. Areas of chipped and peeling paint were observed along the baseboards and door trim in the 3-4 year old classroom. 2. Chipped and peeling paint were observed on the frame of a large swing set outdoors. 3. Protruding nails were observed in various areas of a wooden fence on the toddler playground. 4. Splintering wood and peeling paint were observed on the wooden fence barrier between the large playground and the toddler playground. 5. Frayed carpeting was observed in a room where children nap. Plan of Correction: We plan to close during the upcoming holidays to renovate areas of the program.
Standard #: 22VAC40-185-280-B Description: Based on observations, hazardous substances were not kept in a locked place. Evidence: 1. Disinfectant wipes were stored in an unlocked upper cabinet in a room where infants sleep. 2. A bottle of hand sanitizer was stored on a low shelf in the 3-4 year old's classroom. Plan of Correction: Hand sanitizer removed from shelf during inspection. We will store all cleaning supplies in a locked area.
