Technical Assistance:
Reviewed with center manager the following: should get more staff trained in MAT, there is only one currently who opens and closes the center, purchase a box to store medication for easy evacuation, allergy action plans should be confidential and can be posted inside teacher cabinets. Discussed concerns with placement of the changing table in the Twos class. The table is not currently connected to water but when it is, you can not properly supervise the children when changing diapers. Discussed turning the table if the plumping can be arranged. There is still no changing table in the toddler room, they are using the changing table in the adjoining infant room. Discussed concerns and encouraged the Owner to supply a changing table with running water as soon as possible to eliminate the daily transitioning into the infant room for diaper changing.

Comments:
An unannounced monitoring inspection was conducted from 8:40 am-12:30. There were a total of 49 children with 11 staff members present. Children were observed during breakfast time, engaged in group activities, painting, water table and outdoor play time. A selection of staff and children records, medications and other documents were reviewed. Areas of non-compliance are identified in the violation notice. Standard #: 22VAC40-185-70-A
Description: Based on record review, staff records do not have complete information for each staff's reference contacted. Evidence: The five staff records reviewed did not include the firms that were contacted.

Plan of Correction: We will make sure the relationship of the reference is documented.

Standard #: 22VAC40-185-270-A
Description: Based on observation, areas of the center are not maintained in a safe condition. Evidence: Cribs were observed folded up and leaning against a wall in the mobile infant class.

Plan of Correction: We will open the cribs and positioned them in the room.

Standard #: 22VAC40-185-510-C
Description: Based on a review of medications, the center has not included written authorization from the child's physician for a long term medication. Evidence: Child #1 medication was dated 8/13/18 and still on site this date.

Plan of Correction: I will return the medication to the parent today.

Standard #: 22VAC40-185-510-G
Description: Based on a review of medication, the center has a medication that is not within accordance with the prescriber's instructions pertaining to dosage, frequency, and manner of administration. Evidence: Child # 2 medication form is completed by the physician to administer 0.15 mg dosage of medication. The pharmacy prescription label and medication is 0.1 mg dosage.

Plan of Correction: We will have the parent review the medication form with the physician.
