15 children were in care with two staff. The age range of the children was five years to ten years. Children were observed during free play and snack. Eight children's and two staff records were reviewed, Inspection time 4:00pm-5:30pm
Standard #: 22VAC40-185-60-A Description: Based on the review of eight children's records not all were complete. Evidence: Child B's record was missing the father's work number. Plan of Correction: I will ask the parents for the missing information.
Standard #: 22VAC40-185-280-B Description: Based on observation the center failed to keep all hazardous substances in a locked place. Evidence: One bottle of hand sanitizer was on the stage accessible to the children. LI observed one child use the hand sanitizer. The bottle was labeled with keep out of the reach of children warning label. Plan of Correction: I will keep the hand sanitizer locked.
Standard #: 22VAC40-185-500-A Description: Based on observation not all children washed hands after eating. Evidence: No all children washed hands after eating snack. Plan of Correction: All children will wash hands after snack.
Standard #: 22VAC40-185-510-C Description: Based on review staff did not follow produces for expired medications. Evidence: Child A's epinephrine injector stored at the facility has a expiration date of 4/14/2018 Plan of Correction: I will check all medications before accepting from the parent. I will ask parent for a new medication.
