Description: Based on review of six children's records not all had a physical examination with in 30 days of enrollment. Evidence: Child A with a start date of 4/23/18 did not have documentation of a physical.

Plan of Correction: I will contact parent.

Standard #: 22VAC40-185-60-A
Description: Based on review of six children's records not all were complete. Evidence: Child A's record is missing documentation of previous day care, insurance policy number, mother's home and work information. Child's start date is 4/23/18

Plan of Correction: We will ask parent for the missing information.

Standard #: 22VAC40-185-500-B
Description: Based on observation the center did not have a nonabsorbent surface for diapering. and were not using a covered diaper pail. Evidence: The diaper pad had holes in the surface making it absorbent. The step on diaper pail was missing the lid

Plan of Correction: Corrected during the inspection. A new diaper pad and pail were brought to the center.
