An unannounced monitoring inspection was conducted on November 1, 2018 from 9:15am until 11:00am. Upon arrival to the facility, there were 64 children present with 13 staff members. The children were observed participating in age appropriate activities, outdoor play, hand washing and diapering. A sample of 6 children's records and 6 staff records were reviewed. The licensing inspector toured the facility and reviewed supervision, emergency supplies, practice drills, nutrition, physical plant, required postings, and administration. There was a violation cited in the area of physical plant. An exit meeting was conducted with the facility director prior to the end of the inspection.

Standard #: 22VAC40-185-290-3 Description: Based upon observation and interview the provider did not ensure that the electrical outlets located inside of the facility had protective covers to prevent potential injuries of children. Evidence: 1. During the inspectors tour of the facility it was found that an electrical outlet located in the toddler room of the facility did not have a protective cover. a. The electrical outlet without a protective cover could be a potential hazard to children. b. There were children in the toddler room during the time the violation was observed. c. The staff director toured the toddler room with the inspector and also identified and confirmed the violation. Plan of Correction: Will cover electrical outlet with protective covers.
