An unannounced monitoring inspection was conducted from 9:05 am-10:30am. 13 children were in care with 2 staff members present. Children were observed during engaged in group activities using a variety of table top toys, using the restroom and washing hands after, cleaning up toys and lining up to go outside. Staff had an effective system in place to ensure children followed rules for the playground. 3 staff and 5 children records and medications were reviewed. Standard #: 22VAC40-185-160-C Description: Based on record review, the center did not obtain subsequent tuberculosis (TB) screening for staff every two years. Evidence: Staff # 1's most recent TB expired in June 2018. A copy of a subsequent screening was not available for review at the center. Plan of Correction: I will get the copy and keep in staff file.
Standard #: 63.2-1720.1-B-2 Description: Based on record review and staff interview, the center did not obtain fingerprint results for staff. Evidence: Staff # 1's record did not the results for Field Print fingerprints. Staff # 2 stated that she thought the previously done fingerprints through another agency would be acceptable. Plan of Correction: I will schedule the appointment.
