A Subsidy Health and Safety inspection was conducted on 10/30/2018 from 10:05am to 4:45pm to review supplemental health and safety requirements. The children were observed playing on the playground, participating in story time and singing. A formal observation was completed with the infant and three year old classrooms. The menu was posted. Interviews were held with staff throughout the inspection, and the inspector interacted with the children in each classroom when appropriate. All classrooms and playgrounds were inspected today as well as the mini buses.
Ten children?s records and ten employee records were reviewed during this inspection. Medication is being administered and was reviewed. Twelve injury reports were reviewed as well as the center?s injury prevention plan which was updated 09/4/2018.
The center's first aid kit and emergency supplies were inspected.

A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. A violations was found during the supplemental subsidy health and safety inspection requirements and is identified on the violation notice.
New subsidy requirements became effective on October 17, 2018. The facility has not yet complied with the following requirement(s): [22VAC40-665-540 Tuberculosis screening is now required before coming in contact with children and documentation must have been completed within the last 30 calendar days of the date of employment.] [22VAC40-665-770 Procedures for the continuity of operations to ensure that essential functions are maintained during an emergency must be included in the emergency response plan.] The facility is to review the new requirements and work with their assigned inspector to ensure future compliance.

Standard #: VENDSUB-000-006-B-11 Description: Based on record review, the center did not ensure to obtain a written allergy care plan for each child with a diagnosed food allergy. Evidence: 1. The record of child #1 (start date: 3/9/15) contained documentation of a written allergy care plan that was not signed by a physician. 2. The record of child #7 (start date: 8/30/18) contained documentation of a written allergy care plan that was not signed by a physician. 3. The record of child #8 (start date: 6/18/18) did not contain documentation of a written allergy care plan. Plan of Correction: No medications will be accepted at the center without an allergy care plan.
