Comments:
An unannounced mandated inspection was conducted on 11/1/2018. A walk through of the inside and outside of the program was conducted. The inspector arrived at 11:00 A.M. and departed the program at 1:30 P.M. There were 84 children present during the inspection. The inspector reviewed eight staff's and children's records. The children were observed during: free play, bathroom/handwashing, napping, eating lunch. Discussion with directors: the three required agreements, new requrements for subsidy.

Description: Based on review of eight staff's records, the center failed to ensure that all staff complete the sworn statement prior to the documented start date. Evidence: 1. All staff records reviewed had a sworn statement that was signed on 9/4/2018 and a documented start date of 9/3/2018. The official day for all staff to start work was on 9/4/2018. 9/3/2018 was a holiday.

Plan of Correction: The center will make sure that all staff's records will reflect the correct start date vs. hire date with the new owner.
