Comments:
An unannounced monitoring inspection was conducted from 10:30 am - 12:00 pm. There were 44 children in care being supervised by 3 staff members. The children were observed participating in table top activities, crafts, and preparing for lunch. Lunch served today is cheeseburger on bun with lettuce and pickle, yogurt with sliced fruit, french fries, orange wedges, and milk. Three staff records were reviewed. Ten children's records were reviewed and in compliance. Six medications were reviewed and in compliance. The most recent emergency escape drill was conducted 10/16/2018 and the most recent shelter in place drill was conducted 09/21/2018. Standard #: 22VAC40-185-160-A
Description: Based on review of three staff records it was determined that not all staff records contained documentation of a negative TB (tuberculosis) screening within 21 days of employment. Evidence: Staff A (start date: 10/08/2018) did not have documentation of a TB screening on file.

Plan of Correction: Staff A is scheduled for a TB screening today.
