An unannounced monitoring inspection was conducted from 8:30 am-11:00. There were a total of 61 children with 12 staff members present. Children were observed arriving to the program, eating snack, story, music and movement. A selection of staff and children records, medications and other documents were reviewed. The inspection was reviewed with the new Director. Areas of non-compliance are identified in the violation notice. Standard #: 22VAC40-185-70-A Description: Based on record review, a staff's record is not complete with documentation of 2 or more references. Evidence: Staff #1, date of hire 10/26/18 has documentation one only one reference. Plan of Correction: We will get one more reference.
Standard #: 22VAC40-185-510-G Description: Based on medication review, one child's medication on the prescription label is not in accordance with the prescriber's instructions pertaining to dosage, frequency, and manner of administration. Evidence: Child #1 medication form, signed by the physician says to administer Epipen 0.15 mg. The prescription medication is labeled, Auvi Q, 0.1 mg. Plan of Correction: We will speak with the parent to get a new form with the correct information.
Standard #: 22VAC40-185-550-D Description: Based on a review of emergency evacuation drills, the center has not conducted monthly practice evacuation drills. Evidence: A fire evacuation drill was not conducted in October 2018. Plan of Correction: We will do monthly fire drills.
