VENDSUB-000 STAFF QUALIFICATIONS AND TRAINING
The Licensing Inspector conducted an unannounced complaint inspection on 9/27/18 in response to a complaint that was received in the Licensing office on 8/27/18 concerning the reporting of serious injuries, supervision, and nutrition. Observations were conducted, staff were interviewed, and documentation was reviewed during the investigation. Based on the information gathered during the investigation, the evidence supports the complaint and is deemed to be valid. The area of non-compliance associated with the complaint is on the attached violation notice.
Standard #: VENDSUB-000-009-2 Description: Based on information gathered during the investigation, it was determined that the vendor did not ensure that the department's representative is informed as soon as practicable, but not to exceed two business days, of any serious injury to a child while under the center?s supervision. Evidence: 1. On 8/22/18, child #1 (10 years old) was picked up by a caregiver at 5:30pm. The caregiver noticed that child #1 could not use his right hand when he was trying buckle his seat belt, so the caregiver called child #1's mother. 2. The parent of child #1 called the center to inquire about the injury to her child and inform the center that she was on her way to the Urgent Care medical center to receive treatment for the injury. Staff #3 (Assistant Director) stated she did not know anything about the injury. Child #1 told his mother that he jumped off the play structure on the School Age Playground while running from another child. 3. Child #1 returned to care on 8/23/18 , at this time the mother asked again if they had any information on how her child was injured, and again the answer was "no". In addition, she notified the center again that child #1 received medical treatment on 8/22/18, and was diagnosed with a fractured right wrist. 4. When staff #5 (Program Director) was asked by the Licensing Inspector if the injury had been reported had been reported to Licensing within two business days, he stated "no". 5. A report of serious injury was received on 9/28/18 from the facility, the day following the complaint inspection. Plan of Correction: The facility responded: All serious injuries will be documented and sent to our Licensing Inspector within two day in accordance with policy.
