SHSI requires that confirmed food allergies have a plan of action from the physician.
A Subsidy Health and Safety inspection was conducted on November 5, 2018 to review supplemental health and safety requirements. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.
Standard #: VENDSUB-000-006-B-11 Description: Based on record review of child #3's file, the center did not obtain a written allergy care plan for this child with a diagnosed food allergy. Evidence: 1. Child #3's physician report has a documented food allergy. The parent also recorded the child has a food allergy. The physician did not write a plan of action in the event the child has a suspected or confirmed allergic reaction to the food. Plan of Correction: We will discuss with the parent.
Standard #: VENDSUB-000-012-B Description: Based on interview and record review, the center was not able to provide documentation of Virginia Pre service training for staff #2. Evidence: 1. Staff #2 started at this location in October but has been with program for over 3 months. 2. Program had training documents and First Aid/CPR certification but did not have certificate to verify Pre Service training was completed. Plan of Correction: Will request the document for the other location.
