Comments:
An unannounced monitoring inspection was conducted today with the area manger. Two schools were combined at this one site for teacher work day. Fifty-one children were present with five staff. The children were split into two groups at the beginning of the inspection with one group making "slim" and the second group playing the ball game, "four corners" in the gym. Medications for eleven children were inspected as well as five staff files and 7 children's files. Arrival time: 2: 05pm Departure time: 3: 30 pm

Violations:
Standard #: 22VAC40-185-510-A
Description: Based on a review of eleven medications and an interview with the area manager, it was determined that the center is not following its written medication policies for expired medications. Evidence: Child A's Epipen expired in 9/18 and child B's Epipen expired in 5/18. Both expired medications remain on site.

Plan of Correction: Parents have been contacted and we will obtain the requested information by the end of the week.

Standard #: 22VAC40-185-510-G
Description: Based on a review of medications for eleven children and an interview with the area manager, it was determined that medication for one child was not maintained in the original, labeled container. Evidence: The Epipen for child B did not have a prescription label from the pharmacy.

Plan of Correction: Parents have been contacted and we will obtain the requested information by the end of the week.
