A Subsidy Health and Safety inspection was conducted on (insert date) to review supplemental health and safety requirements. An unannounced monitoring inspection was conducted today from approximately 9:30am-12:45pm. There were 40 children in the direct care and supervision of 13 staff. A sample of 5 children?s records and 5 staff records were reviewed. The children were observed learning colors, types of food, and reading books with teachers. Zero new staff have been hired since the last inspection. Areas of non-compliance are identified in this report. The exit interview was conducted with the director. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Standard #: VENDSUB-000-012-B Description: Based on record review, all staff did not complete the Virginia Preservice training. Evidence: Staff A did not have documentation to prove that she has completed the Virginia Preservice training. Plan of Correction: Staff A is from a neighboring center, I will retrieve the required documents.
Standard #: VENDSUB-000-012-F Description: Based on record review, all staff did not complete CPR and First Aid training. Evidence: Staff A and B did not have documentation of complete First Aid and CPR trainings. Plan of Correction: Both staff are from a neighboring center, I will retrieve the required documents.
