Comments:
An unannounced monitoring inspection was completed today. This is a 2 month follow-up visit during the Provisional licensure period. Areas of the center, 5 staff files, 5 children's files, and emergency procedures were inspected between 12:00-1:20pm. There were 97 children under the supervision of 28 staff. For lunch, the children were served veggie tacos with brown rice, mandarin oranges and milk. Should you have any questions, please contact me at mary.wizbicki@dss.virginia.gov.

Violations:
Standard #: 22VAC40-185-70-A
Description: Based on file review, 2 out of 5 staff records do not have written information to demonstrate that the individual possesses the orientation training required by the job position. Evidence 1. Staff #2 and #3 do not have documentation of having received orientation training.

Plan of Correction: These staff received orientation training, but it was not documented. The orientation form will be completed.
