Licensing inspector arrived at 10:30 am and departed at 1:30pm to conduct a licensing inspection and a Subsidy Health and Safety inspection. At the time of the tour there was forty children in care with nine staff . Children were observed playing in stations, recognizing and matching colors with objects, eating lunch and nap time. The sample size was five staff records and five children records.

Standard #: 22VAC40-185-60-A Description: Based on 1 of 5 children's records reviewed, the licensee did not ensure each child enrolled had a record which contained all required information. Evidence: During the inspection conducted on November 6, 2018, the following information was missing in the children records: Child #1 was missing one of the parents work phone numbers. Child #2 was missing one of the parents work phone numbers and one emergency contact complete address that included city , state and zip code and need one additional emergency contact other than parents. Child #3 was missing two emergency contact addresses. Child #4 was missing one emergency contact address. Child #5 was missing one parents work phone number. Plan of Correction: Not available online. Standard #: 22VAC40-185-90--A Description: Based on record review, the licensee did not ensure a written agreement between the parent and the center were in each child's record by the first day of the child's attendance. The agreement shall be signed by the parent. Evidence : On November 06, 2018, child #3 did not have agreements signed . Plan of Correction: Not available online. Standard #: 22VAC40-185-270-A Description: Based on observation, the licensee did not ensure all areas and equipment of the center, inside and outside, were maintained in a clean, safe and operable condition. Evidence: During the inspection conducted on November 06, 2018 , the licensing inspector observed the following inside and outside the building: - In the three and four year old bathroom , the toilet lid was missing and a cardboard was taped on the toilet replacing the the lid. - In the four year old classroom bathroom, the divider is rusting and there was red rust flaking on the floor. - Outside on the preschool playground area, the wooden door frame at the bottom is deteriorating the wood is broken and there is whole on the frame, peeling paint on the building, top of the swing is rusting a red residue comes off the top of the bar, the wood that supports a white board on the playground composite is splintering, deteriorating and sharp and a black trash can lid broken with sharp edges. - Outside on the infant, toddler and two year playground, paint is peeling off the building.
