An unannounced monitoring inspection was conducted in the Center on August 29, 2018. The Licensing Inspector observed 11 children and 3 staff present in the Center. As reported, there are currently 13 children enrolled in the program. Two staff were observed actively supervising the children's activities. An additional staff was also present whom was observed preparing foods for the lunch meal. The children participated in a variety of activities which included circle time, music and selected centers. The LI observed hand washing procedures and the lunch meal. The children were also observed during outdoor play and dismissal at the end of the program day. The classroom was clean and organized with age appropriate materials. Staff were observed interacting with the children. The children appeared to enjoy their day and the activities. A sample of 5 children's records and 3 partial staff records was reviewed. Complete staff records were reviewed on September 18, 2018 at the administrative office. Documentation of the annual fire and health inspections was current. The first aid kit and posted emergency numbers were observed. A prescribed medication was observed in the Center. The on-site inspection was conducted from approximately 11:20am to 2:50pm. Findings of the inspection were reviewed with the Lead Teacher at exit. Standard #: 22VAC40-185-150-B Description: Based on reviewed documentation, the Center failed to ensure each child's immunization record was signed by a physician, his designee, or an official of a local health department. Evidence: One of 5 reviewed immunization records was not signed, the immunization record for child #5 contained page 1 of 2. There was no signature page available for review. Plan of Correction: The responsible staff will ensure each child's record contains a current immunization record which has been signed by the appropriate medical professional.
Standard #: 22VAC40-185-60-A Description: Based on a sample review of children's records, the Center failed to ensure records contained all required information. Evidence: The work phone numbers and place of employment for each parent who has custody was not complete in one of five reviewed records (child #3). Plan of Correction: The responsible staff will review records with the parent of each child to ensure all information is complete.
