63.2 Facilities & Programs.
An unannounced monitoring inspection was conducted on November 5, 2018 between the hours of approximately 10:10 a.m. and 1:20 p.m. Twenty-five (25) children were enrolled and nineteen (19) children were in the care of five staff members, in four classrooms, during the on site inspection. One additional staff member was present and assisted, as needed. Five children's records and six staff records were reviewed and/or updated. No medication was on site/administered, as is the policy of this child day center. Posted information and available documents were observed/reviewed. Lunches and snacks were provided by the children's parent(s). The facility was observed to be clean, well maintained and supplied with a variety of age/stage appropriate play and learning materials/toys. Program observation was completed. Activities observed included: indoor play (toys, books and fingerpainting); free choice play in the gross motor room; diapering and handwashing procedures; lunch; naptime; infant care and pick-up/departure of children. Staff/child interactions, behavior guidance and daily routines/transitions were also observed. Staff members were observed to be positive and supportive in their interactions with the children in their care. The program was observed to be in substantial compliance with the licensing standards; exceptions to compliance were noted on the accompanying Violation Notice. Standard #: 22VAC40-185-160-A Description: Based on review/update of six staff records and discussion with staff on November 5, 2018, the program failed to ensure one staff member submitted documentation of a negative tuberculosis (TB) screening no later than 21 days after employment. Evidence: 1. Review of staff #1's record, on this date, determined that a TB screening/exam report was dated more than 21 days after employment. 2. Staff #1 confirmed the date of employment (9/17/2018) and the date of the TB screening/exam report. Plan of Correction: In the future, TB screenings/exam reports will be obtained no later than 21 days after employment.
Standard #: 22VAC40-185-550-D Description: Based on review of the emergency evacuation drill log and discussion with staff on November 5, 2018, the program failed to implement a monthly practice evacuation drill in September 2018. Evidence: 1. Review of the emergency evacuation drill log, on this date, determined that a practice drill was not conducted in September 2018. The program began operation on September 17, 2018. 2. Staff #1 verified that an emergency evacuation drill was not conducted in September 2018. The first practice drill was conducted on October 19, 2018. Plan of Correction: In the future, monthly practice evacuation drills will be implemented.
