Discussion was held on the topics of current enrollment, current staffing, sanitation tablet within an upper cabinet located in the infant room, and a crib located within the infant room in which the manufacture date could not be determined. The crib had a label of "do not use" on it and it was not in use at the time of inspection.
The following inspection took place between the hours of 10 am through 3 pm. Today, I met with the Staff of Your Child's Place located in Vienna, Virginia. At the start of this unannounced renewal inspection there were a total of 66 children with 11 staff (within the 2 buildings); good ratios in all 7 rooms. A total of 6 children?s files and 6 staff files were reviewed. The children were engaged in the following: freeplay, dance, music, play-dough, learning about animals, letters, numbers, playing with blocks, large motor activities on the two center playgrounds, lunch, nap time, and other organized activities. Standard #: 22VAC40-185-210-A-4-b Description: Based on review, 1 of 6 staff files reviewed indicated that qualifications could not be established for a current position. Evidence - 1. On the date of inspection (11/08/2018), the training hours for Staff #3 was less than 24 hours within the past 6 months. 2. The training period began in October 2018. Staff #3 currently has 21 hours in total. 3. The start date for Staff #3 was listed as 09/14/2015. Plan of Correction: In order to complete all required training hours, Staff #3 shall complete at total of 40 hours of training; 24 for her promotion, and 16 for her annual training.
Standard #: 22VAC40-185-270-A Description: Based on observation, some areas and equipment, both inside and outside the center, were not maintained in a safe condition. Evidence - 1. On the date of inspection (11/08/2018), chipped paint was observed by the changing table in the infant room. This area was accessible to the children in care. 2. Within the "Froggy" room, the area of the wall by the sink showed signs of wear to the point that the drywall was damaged. 3. On the younger playground, an exposed nail head was visible beside the top part of the yellow slide. This protruding nail may pose as a "snagging point." 4. On the older playground, a closed manhole cover had a loose table-top over it. This table-top was damaged with a hole in its center. Plan of Correction: 1. The center plans to take a week off during the winter holidays. Maintenance issues shall be addressed at that time. In the meantime, the area shall be scraped down. 2. Same as above. 3. The nail head shall be hammered down. 4. The loose table-top shall be thrown away.
Standard #: 22VAC40-185-280-B Description: Based on observation, hazardous substances such as cleaning materials were kept in a locked place using a safe locking method that prevents access by children. However, the key was still accessible to the children. Evidence - 1. On the date of inspection (11/08/2018), within both the Froggy and Butterfiles rooms the key was observed still within the lock located on the cleaning supplies cabinet. 2. Children were in both rooms at the time of this observation. Plan of Correction: Corrected on the date of inspection.
Standard #: 22VAC40-185-500-B Description: Based on observation, the diapering surface was used for more than diapering or the cleaning of children. Evidence - 1. On the date of inspection (11/08/2018), rubber gloves (unboxed) were observed in a large pile underneath the changing pad. 2. In addition, an opened back of stickers were also observed stored underneath this same changing pad. 3. The amount of time that these items were exposed while stored in this area was undetermined. Plan of Correction: Corrected on the date of inspection. These items were moved to another area.
Standard #: 22VAC40-185-510-C Description: Based on review, 1 of 2 medications reviewed indicated that the center, for nonprescription medication, were not consistent with the manufacturer's instructions for the age of the child. Evidence - 1. On the date of inspection (11/08/2018), the medication on site for Child #7 stated on the original box: "children 2 to 5 years, do not use unless directed by a doctor. 2. The center obtained a Medication Consent Form signed by the Parent of Child #7, but not signed by a doctor. Plan of Correction: The medication shall be returned to the parent with a form for the child's doctor to sign. Once this signed form is completed, it shall be obtained and placed on file.
Standard #: 22VAC40-185-540-C Description: Based on review, the first aid kit located in building YCP2 did not contain all of the required items. Evidence - 1. On the date of inspection (11/08/2018), the following items were not observed: tape, and (2) two triangular-bandages. 2. This first-aid kit was stored in a locked place under one of the sinks. Plan of Correction: Items were moved into this first aid kit, such as the tape, and (1) one triangular-bandage. The other bandage shall be ordered.
