Comments:
An unannounced monitoring inspection was conducted today from 4:40pm-6:00pm. A total of 62 children were present with 4 staff. Children were observed engaged in a variety of individual and group activities. A sample of 6 children's records, 5 staff records, drills, medications and injury reports were reviewed. Violations:
Standard #: 22VAC40-185-320-B
Description: Based on observation, the water temperature for sinks used by children exceeded 120 degree Fahrenheit. Evidence: The sinks in both Classroom A and B registered at 122 degree Fahrenheit.

Plan of Correction: We will bring it to the attention of the school administrators.

Standard #: 22VAC40-185-350-E-6
Description: Based on observations, ratios of one staff to every 20 children were not maintained. Evidence: Upon arrival, the inspector observed 29 children under the care and supervision of 1 staff for approximately five minutes.

Plan of Correction: We will work to maintain our ratios.
