Comments:
An unannounced monitoring inspection was conducted today from approximately 11:45am-3:50pm. There were170 children in the direct care and supervision of 25 staff. A sample of 10 children?s records, 5 staff records, 5 minor injury logs and 5 medications were reviewed. The children were observed eating snack, napping, hand washing, reading books, during bathroom procedures, singing and engaged with staff. 5 new staff have been hired since the last inspection. Violations:
Standard #: 22VAC40-185-160-A
Description: Based on records reviewed, each staff member hired did not have a tuberculosis screening completed within 12 months prior to being hired. Evidence: Staff A was hired 09/10/2018 and their tuberculosis exam was completed 08/25/2017.

Plan of Correction: I will have her complete a new TB screening.

Standard #: 22VAC40-185-40-J
Description: Based on interview with staff, injury prevention procedures were not maintained and updated annually. Evidence: Staff A and B stated that the center had not developed an injury prevention plan.

Plan of Correction: We are in the process of putting a plan together.

Standard #: 22VAC40-185-60-A
Description: Based on record review, children's records were incomplete. Evidence: 10 of 10 children's records reviewed were missing required information. 1) Children A-J were missing school previously attended. 2) Child A was missing father's work phone number. 3) Child B was missing father;s residential information, father's work information and physicians phone number. 4) Child C was missing father's employment information and physicians phone number. 5) Child D was missing father's employer phone number. 6) Child E was missing the addresses of 2 of 2 required emergency contacts.

Plan of Correction: Parents will update information by this Friday.

Standard #: 22VAC40-185-70-A
Description: Based on record review, staff records were incomplete. Evidence: 3 of 5 records reviewed were missing required information. 1) Staff A was missing health information. 2) Staff B and C were missing 2 of 2 required references.

Plan of Correction: I will get the references for staff when HR returns.

Standard #: 22VAC40-185-550-M
Description: Based on record review, minor injury reports were incomplete. Evidence: Of 5 minor injury records reviewed the following was missing: 1 was missing future action to prevent injury, method, date and time in which parent was notified of the injury.

Plan of Correction: I will make sure staff complete the entire form.
