Comments:
An unannounced monitoring visit and facility tour was conducted from 11:05am-12:55pm. At the time of inspectors' arrival there were 78 infant to kindergarten aged children in care with 15 staff members and 2 administrators. Children were observed participating in lunch, indoor play, transitioning between activities, diapering, handwashing, nap, and educational activities. A sample of 7 children's records and 5 staff records were reviewed. Medications and injury reports were also reviewed. The areas of noncompliance are detailed in the violation notice and were discussed with the director during the exit meeting at the conclusion of this inspection.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation it was determined the center did not ensure that all areas and equipment of the center, inside and outside, were maintained in a safe and operable condition. Evidence: Outside on the playground there were multiple areas within the fall zones of the poured in place resilient surfacing for the play equipment that were cracked or in disrepair; presenting potential injury hazards.

Plan of Correction: Repairs to the poured in place surfacing will be completed.

Standard #: 22VAC40-185-570-E
Description: Based on observation and interview it was determined the center did not ensure prepared infant formula is dated and labeled with the child's name. Evidence: In the infant room refrigerator: 1. The bottles for child #6 did not contain a date. 2. The bottles for child #8 did not contain a date. The staff confirmed there was no date on the bottles.

Plan of Correction: Staff will be reminded to ensure the child's name and date are on all bottles.
