An unannounced monitoring visit and facility tour was conducted from 9:50am-11:45am. At the time of inspectors' arrival there were 92 toddler to school aged children in care with 11 staff members and 1 administrator. Children were observed participating in instructional time in the classroom, lunch, outdoor play, transitioning between activities and toileting and hand washing. A sample of 9 children's records and 9 staff records were reviewed. Medications and injury reports were also reviewed. Areas of noncompliance were discussed with the center administration at the conclusion of this inspection and are detailed on the violation notice.
Standard #: 22VAC40-185-130-B Description: Based on record review it was determined the center did not obtain documentation of additional immunizations once every six months for children under the age of two years. Evidence: The record for child #5 did not contain updated immunization documentation once every six months until the child turned 2 years old. The documentation available was dated 3/16/17, child #5 was not two years old at that time. Plan of Correction: Director will request updated immunization documentation from the parents.
Standard #: 22VAC40-185-260-A Description: Based on record review and interview it was determined the center could not provide the licensing representative an annual fire inspection report from the appropriate fire official having jurisdiction. Evidence: 1. The center did not have a current fire inspection. 2. The fire inspection was dated 11/6/2017. 3. The Director confirmed this was the most recent fire inspection. Plan of Correction: The Director has contacted the fire department to request a fire inspection.
Standard #: 22VAC40-185-270-A Description: Based on observation it was determined the center did not ensure all areas and equipment of the center, inside and outside, were maintained in a safe condition. Evidence: 1. In staff #10's classroom there was a radio on the counter top of the teacher's cabinet, the cord was dangling over the side and accessible to children in care presenting a potential fall hazard. 2. There were twenty-two 2-3 year olds in this classroom during this inspection. Plan of Correction: Staff will be reminded to not leave cords dangling in the classrooms.
