An unannounced monitoring inspection and facility tour was conducted from 1:45pm-3:35pm. At the time of inspectors' arrival there were 47 infant to preschool aged children in care with 8 staff members and 2 administrators. Children were observed participating in nap, educational activities, handwashing and toileting, and transitioning between activities. A sample of 6 children's records and 6 staff records were reviewed. Medications and injury reports were also reviewed. The areas of noncompliance were discussed with the directors during this inspection and are detailed on the violation notice.

Standard #: 22VAC40-185-160-C
Description: Based on record review it was determined that the center did not ensure that at least every two years from the date of the first initial screening or testing staff members obtain and submit the results of a follow-up tuberculosis screening. Evidence: The record for staff #6(hired 6/2/15) did not contain an updated TB test or screening. The available documentation was dated 7/3/16.

Plan of Correction: Staff will provide an updated TB test for the record.

Standard #: 22VAC40-185-260-A
Description: Based on record review and interview it was determined the center could not provide to the licensing representative an annual fire inspection report from the appropriate fire official having jurisdiction. Evidence: 1. The director did not have a current fire inspection. 2. The fire inspection available was dated 12/16/15. 3. The director confirmed they did not have a current fire inspection available during this inspection.

Plan of Correction: Center will contact the Fire Marshall's office to request a fire inspection.

Standard #: 22VAC40-185-270-A
Description: Based on observation it was determined the center did not ensure that all areas and equipment of the center, inside and outside, were maintained in a clean, safe and operable condition. Evidence: 1. In the Pooh & Friends, Little Rascals, and Sesame Street classrooms there was peeling paint on the walls and windows. 2. In the Sesame Street, Little Rascals, and Pooh & Friends classrooms/restrooms the cove base is separating from the walls. 3. In the restroom in the Pooh & Friends class there were two 1/2" holes in the wall behind the toilet. 4. In the restroom in the Little Rascals class there was exposed rust on the bathroom partition and a 1" hole behind the door. 5. On the outdoor playground there was: -a 4x6" hole in the concrete to the right of the exit door from the Pooh & Friends class -exposed rust on the tricycles and See-Saw Rocker -cracked Little Tikes plastic picnic table -a hole in the exterior of the building exposing Styrofoam and fiberglass All of these areas were being utilized by children in care during this inspection and presenting potential injury hazards to the children in care.

Description: Based on record review and interview it was determined the center did not ensure that all employees completed the required National Criminal record check by September 30, 2018. Evidence: 1. The records for staff#1(hired 8/8/16), staff #2(hired 6/2/15), staff #3(hired 3/28/17), staff #4(hired 7/5/15), staff #5(hired 9/14/15), and staff #6(hired 6/2/15) did not contain documentation of National Criminal record checks. 2. Staff #1, #2, #3, #4, #5, and #6 were working during this inspection. 3. The director confirmed the staff had not completed the National Criminal record checks.
