The licensing inspector conducted an unannounced renewal inspection on Wednesday, November 14, 2018 from 02:30pm to 05:38pm. The children were observed getting up from rest time and eating snack. Snack consisted of cheez it crackers and apple juice. Some of the children were at a soccer enrichment program. All classrooms were inspected today as well as the facility?s mini bus. Nine children?s records and nine employee records were reviewed during this inspection. The center's first aid kit and emergency supplies were inspected. Last emergency drill: 10/30/2018 Last shelter-in-place drill: 06/01/2018 and 09/25/2018 Last fire inspection: 10/31/2018 Last health inspection: 11/02/2018 The following child to staff ratio was observed: Infants-11:3 Infant II-8:2 Toddlers-10:2 Two's-21:3 Preschool-19:2 Pre-K-19:2 School Age-8:1 If you have any questions about this inspection, please contact the licensing inspector, Roshon Casey, at (804) 662-9760. PROVIDER: Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word ?corrected? is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Standard #: 22VAC40-185-270-A Description: Based on observation on 11/14/2018, the center did not ensure that equipment of the center is maintained in a safe condition. EVIDENCE: There was a protruding screw on the sliding board equipment on the toddler playground. The screw protruded out approximately 3 inches and was located on the lower part of the equipment at a child's eye level. Plan of Correction: Not available online.
