22VAC40-191 Background Checks (22VAC40-191)

An unannounced monitoring inspection was conducted from 3pm through 4:30pm. There were 37 children present with seven staff including the owner/program director. Children present range in age from two to ten years old. Children's and staff records were reviewed. Children were observed in teacher and self-directed activities including snack, homework, restroom routines and play. Required postings, annual reports, first aid items, emergency equipment and emergency evacuation drills were reviewed. Violations were identified in the areas of record keeping, physical plant, hand washing and sanitation and appear on the violation notice.

Standard #: 22VAC40-185-60-A
Description: Based on record review and interview the center failed to ensure that each child's record contains all required information. Evidence: 1 - The record for child one does not have documentation that the record was updated annually as required. The documented first date of attendance for child 1 is 5/15/17. a. Child 1 was present for care during the inspection. 2 - Staff 1 confirmed that the record of child 1 does not contain documentation of annual update.

Plan of Correction: Documentation of annual updated information will be obtained and placed in the record of child 1.

Standard #: 22VAC40-185-270-A
Description: Based on observation and interview the center failed to maintain areas and equipment in safe condition. Evidence: 1 - The carpeting at the top of the stairs leading to the school age room has come become loose from the floor board and has created a trip hazard. 2 - There is a dangling electrical cord hanging down within reach of children in the school age room. Child was observed standing behind the dangling cord so that the cord was hanging down under his chin. The dangling cord presents an entanglement hazard.

Plan of Correction: The carpet on the stairs will be replaced or repaired as soon as possible. The dangling electrical cord was secured out of reach of children during the inspection.

Description: Based on observation and interview the center failed to ensure that children's hands are washed with soap and running water or disposable wipes before and after eating meals or snacks. Evidence: 1 - Children in the school age room did not wash their hands after eating afternoon snack. Children were observed as they finished snack and immediately began working on homework and table top play activities.

Plan of Correction: Center management will ensure that children either wash their hands with soap and running water or disposable wipes before and after meals and snacks.

Standard #: 22VAC40-185-560-K
Description: Based on observation and interview the center failed to ensure that tables are sanitized after each use for feeding. Evidence: 1 - The tables in the school age room were not sanitized after being used for afternoon snack. 20 children in the school age room finished snack and immediately began doing homework and playing on the same tables they had been served snack. 2 - Staff 2 stated that the tables were not sanitized after the children ate snack and before using the tables to do homework.

Plan of Correction: Center management will remind and retrain all staff in the necessity to sanitize all tables after meals and snacks.
