Comments:
An unannounced monitoring inspection was conducted from. to 11:30 a.m. to 1:50 p.m. At the time of entrance the following adult to child ratios were observed: Classroom A: 13 children, ages 3 to 4 years old, with 2 staff Classroom B: 9 children, ages 3 to 4 years old, with 2 staff Classroom C: 12children, ages 3 to 4 years old, with 2 staff Classroom D: 8 children, ages 3 to 4 years old, with 1 staff Classroom E: 12 children, ages 3 to 4 years old, with 2 staff Classroom F: 15 children, ages 3 to 4 years old, with 2 staff The records sample size consisted of 9 children?s records. Staff records are housed off site and will be reviewed at a separate inspection. Children were observed dancing and moving to music, cleaning up from learning centers, eating lunch which consisted of cheeseburgers on a bun, pineapple chunks, coleslaw and milk. Children were observed washing hands and settling on cots for nap period. Restroom procedures and table sanitizing were also observed. Medication administration was not observed. However, a sampling of 5 medications were reviewed with regard to storage, labeling, written authorization and required documentation. MAT (medication administration) Training and First Aid / CPR training was posted in the classrooms. Staff report no accidents or injuries requiring first aid or medical attention since the last inspection. Thank you for your time during today's inspection. Description: Based on observation, the licensee did not ensure that electrical outlets, when not in use and located in areas where children could access them, have protective covers that are of a size that cannot be swallowed by children. Evidence: The licensing inspector observed one uncapped outlet on a two-outlet panel above the handwashing sinks in classroom C. The outlet was not in use and could be accessed when children stand on a step platform located in front of the sinks while washing their hands.

Plan of Correction: Staff placed a protective cover on the outlet when this was brought to their attention by the inspector.
