22VAC40-191 Background Checks (22VAC40-191)

An unannounced monitoring inspection was conducted today November 15, 2018 as a follow-up to the issuance of a conditional license. The inspector arrived at 12:10 pm and concluded the inspection at 3:00 pm. Upon the inspectors arrival The toddlers and pre-school children were combined combined for nap time. Children were napping with soft music playing. Infants were observed being fed, napping and later practicing sign language. Staff interacted in a positive and nurturing manner. All classrooms, rest rooms, kitchen and hallway area were inspected. The classrooms are equipped with age and stage appropriate materials for literacy, art, fine and gross motor skills and creative play. The facility had the required information posted to include the License, last inspection notice, emergency numbers, daily schedule, evacuation maps and various parent information. The first aide kit and non-medical emergency supplies were inspected and found complete. The inspector interacted with staff and children in care. Three staff records were reviewed during today's inspection along with the records of five children. Staff have current CPR, First aid training and daily Health observations. Today's Lunch consisted of mini corn-dogs, baked beans, oranges, milk. Today's Ratio: (Infants): 3 children with 1 staff. (Toddler-pre-school): 4 children with 1 staff. The last emergency evacuation drill was conducted on 10/23/2018. One of the two required shelter-in-place drills for the 2018 year was documented on 09/17/2018. Standard #: 22VAC40-185-160-A
Description: Based on review of staff records, the facility did not obtain documentation of a tuberculosis screening no later than 21 days after employment and that had been completed within 12 months prior to or 21 days after employing or volunteering. Evidence: Staff #3, hired 08/24/2018, had documentation of a tuberculosis screening dated 08/15/2016, exceeding 12 months prior to employment.

Plan of Correction: Per the facility: I will have the staff obtain a new tuberculosis screening immediately.

Standard #: 22VAC40-185-60-A
Description: Based on review of children's records, the facility did not obtain all required information for each child's record. Evidence: 1. The record of child #1, enrolled 09/11/2018, did not contain the addresses for the two emergency contacts listed. 2. The record of child #4, enrolled 09/28/2018, did not contain the address for one emergency contact listed.

Plan of Correction: Per the director: I will have the parent complete the missing information tonight at pick up.

Standard #: 22VAC40-185-70-A
Description: Based on review of staff records, the facility did not obtain all required documentation for each staff record. Evidence: Staff #3, identified as a program leader, did not have written documentation to demonstrate the individual possesses the education and experience required by the job position.

Plan of Correction: Per the director: I will have staff submit diplomas/ transcripts and finish training hours.
