22VAC40-185 SPECIAL CARE PROVISIONS AND EMERGENCIES.
On 11/5/18, the licensing office received an online report of a serious injury that occurred on 11/2/18 at approximately 5:30pm. The report was related to a child falling in the gym and sustaining injuries to both wrists. Staff and parent interviews were conducted. Staff administered first aid to the child at the time of the injury. Violations related to the incident were cited and listed in the violation notice. Standard #: 22VAC40-185-550-M Description: Based on staff and parent interviews, the center did not maintain a written record of an injury. Evidence: 1. On 11/2/18 at approximately 5:30pm, Child # 1 was playing a game of tag in the school gym with other children and Staff # 1. 2. Staff # 1 stated that Child # 1 was trying to slide on the gym floor. As Child # 1 was sliding he lost his balance and fell. 3. Staff # 1 stated that as Child # 1 fell he landed on both his hands. Staff # 1 noticed that when Child # 1 stood back up he was rubbing his fingers. Staff # 1 stated that when she asked Child # 1 to indicate any area of pain, Child # 1 pointed to the top of his left wrist. 4. On 11/8/18, Child # 1's parent was contacted by the licensing inspector. The parent stated that she had not yet received a written report of the injury from the center. 5. On 11/8/18, Staff # 2 stated that he had written a report and submitted it to his supervisor for review, but a written report had not been provided to the parent and was not maintained at the center. Plan of Correction: A copy of the incident report was given to the parent.
