During the inspection, the licensing representative discussed with administrator: - a medication authorization for an infant that will "expire" 11/18/2018
An unannounced monitoring inspection was conducted today from 11:15am-1:30pm. A total of 81 children were observed in the care of 12 staff. Children were observed during story-time, handwashing and transition to lunch. Infants were observed during bottle feeding and diapering/handwashing. A sample of 5 staff records, 10 children's records, injury reports, medications and emergency drills were reviewed. Areas of non-compliance are documented on the violation notice. Standard #: 22VAC40-185-70-A Description: Based on interview and a review of records, a record was not maintained for 1 out of 5 staff. Evidence: The record for Staff #2 was missing documentation of two or more references, an emergency contact and documentation of staff's education, staff development and experience to qualify them for the position. Plan of Correction: Staff recently transferred from another location. The file is in transit in the mail.
Standard #: 22VAC40-185-280-B Description: Based on observations, hazardous substances were not kept in a locked place. Evidence: 1. 2 bottles containing bleach solution were observed in an upper cabinet that was unlocked in a Toddler classroom. 2. A bottle containing bleach solution was observed in an upper cabinet with a disabled lock in a Two's classroom. Plan of Correction: We will make sure to follow-up behind staff to ensure that substances remained locked.
