Thank you for your time and assistance with the unannounced monitoring inspection conducted today, November 13, 2018. The on-site inspection was conducted from 2:15 p.m. to 6:15 p.m. On this date, there were 38 children in one group and in the care of four to five staff members. The total enrollment for the program was 83 children. Five staff members' records and five children's records were reviewed. The medication and medication paperwork were reviewed for six children. Five staff members and four children were interviewed. The exception to compliance was noted on the violation notice and the risk rating was noted on the supplemental page. Snack consisted of pudding, graham crackers, and juice. Some items observed and reviewed during the inspection included required posted information, first aid supplies, emergency supplies, current health and fire inspections, documentation of evacuation, shelter-in-place and lockdown drills, arrival procedures, hand washing and bathroom procedures, snack, transitions, homework time, large motor games in the gym, music and movement, table games, and dismissal procedures. Standard #: 22VAC40-185-510-C Description: Based on review of medications for six children and verification from staff, the center failed to follow their procedures to prevent a medication from expiring. Evidence: 1. The emergency medication for Child A expired 10/31/2018. 2. The staff did not follow policy of checking the medications every two weeks to prevent medications from expiring. Plan of Correction: Staff called the parent of the child and the parent will obtain a new medication by tomorrow. The medication was taken off the site.
