Thank you for your time and assistance with the unannounced monitoring inspection conducted today, November 19, 2018. The on-site inspection was from 8:55a.m. to 11:30a.m. On this date, there were 17 children present in the Monday, Wednesday, and Thursday four-year-old class and in the care of three staff members. There were a total of 37 children enrolled in the morning preschool program. The violation from the previous inspection was reviewed for compliance. There were five children's records reviewed and six staff records reviewed. There were two staff members interviewed, as well as two children. The classroom was observed. There were no medications being administered, which is the policy of the center. Snack was provided by the center and consisted of graham crackers and apple juice. The exception to compliance was noted on the violation notice and the risk rating was noted on the supplemental page. This week's theme was Thanksgiving and the children enjoyed making their headbands and drums for the upcoming Thanksgiving feast. Some items observed and reviewed included: choice/center time, transitions, small motor activities, circle time, calendar, morning snack, hand washing and bathroom procedures, Thanksgiving art activities, story time, outside walk, and dismissal procedures. Standard #: 22VAC40-185-240-C Description: Based on review of six staff records and verification from staff, one staff member failed to obtain all of the required 16 hours of staff development training for the year. Evidence: 1. The record for Staff D contained documentation that six clock hours of annual training had been obtained from October 2017 to October 2018. Date of hire was 10/19/2018. Plan of Correction: The staff member will make sure to take 10 hours of training as soon as possible and then will make sure to obtain at least 16 hours for the 2018-19 year.
