Comments:
The licensing inspector conducted an unannounced monitoring inspection on Friday, November 9, 2018 from approximately 10:50 am until 12:48 pm determine the center's compliance with licensing standards. During this inspection, the children in care were observed by the licensing inspector in the two classroom settings. The census for today's inspection consisted of children with four staff present. The licensing inspector completed observations of the children in care completing a variety of activities to include transitioning from lunch in the classroom, completing bathroom breaks, participating in centers, and preparing for naptime and napping. The center's bathrooms, and designated classrooms, and equipment were inspected by the licensing inspector. The required posting were reviewed by the inspector. There were five children?s records reviewed by the inspector during the inspection. The center's first aid kit and emergency supplies were inspected. The licensing inspector completed an annual review of staff records at the program onsite location on November 16, 2018. There were five staff records reviewed during this inspection. Description: Based on review of children's record, the program did not ensure to obtain tall of the required information.

Evidence: During the inspection on November 9, 2018, in Child#4's record the father's home address was incomplete.

Plan of Correction: Not available online. Contact Inspector for more information.

Standard #: 22VAC40-185-70-A
Description: Based on review of staff record and interview with staff, the program did not ensure all staff name, address and telephone number of a person to be notified in an emergency which shall be kept at the center.

Evidence: During the inspection on November 9, 2018, the staff emergency contacts for each staff at the facility was not found. Interview with staff confirmed that the required information was not available at the site.

Standard #: 22VAC40-185-260-B
Description: Based on review of health inspection and interview with staff, the program did not ensure an annual health inspection was conducted.

Evidence: During the inspection conducted on November 9, 2018, the last dated health inspection was 05/15/2017 at the facility. Interview with staff revealed an annual health inspection was not conducted for 2018 at this time.

Plan of Correction: Annual health inspection documentation was secured.

Standard #: 22VAC40-185-270-A
Description: Based on observations at the facility, the program did not ensure to maintain all areas in a clean, safe and operable condition.

Evidence: During the inspection conducted on November 9, 2018, the following observations were completed at the facility:
1. There were multiple areas of peeling paint, and visible holes on the wall under the toilet paper dispenser locate in the bathrooms in Classroom 1 and 2.
2. The ceiling light fixtures and fan surrounded by areas of cracked and peeling paint in the Classroom 1 and 2.
3. In Classroom 1, the wooden door frame was broken.
4. There were multiple wooden pieces with visible protruding nails on the ground outside nearby the wooden ramp.
