A self-reported incident was received on September 20 and initiated on the same day. An on site investigation including record reviews and interviews was completed on 11/1/18. Interviews were conducted via telephone as well. The investigation concluded on 11/13/18. A single violation regarding forbidden action was identified and may be reviewed on the violation notice.
Standard #: 22VAC40-185-410-1 Description: Based on interview and written statement the center did not ensure that physical punishment of a child is forbidden. Evidence: 1 - Staff 1 reported verbally and in a written statement that at approximately 11am on 9/20/18 staff 2 "popped" child 1 on the bottom as he walked pasted her to enter the classroom from outside. Then after child 1 sat down on the carpet, staff 2 "plucked" child 1 on the left ear. 2 ?On 11/13/18 in a telephone interview staff 2 stated that she patted six children on the bottom as they entered the classroom including child 1. Additionally, staff 2 stated that she touched child 1 on the ear. a. Furthermore, staff 2 stated that she was trying to get control of the group as they had run away from the classroom door to another playground. She wanted to let the children know that they could not run away from the group. Plan of Correction: Employee 2 was separated on 9/21/18. DM will monitor compliance with VA state and LCG company standards and policies on all regular unannounced bi-weekly visits.
