Description: Based on record review of Staff 31's file, Staff 1 has not obtained a TB Screening every two years. Evidence: 1. Staff #1's TB Screening report states it was conducted on November 10, 2016.

Plan of Correction: I will have the staff member have a screening conducted.

Standard #: 22VAC40-185-280-B
Description: Based on observation, the center did not store all cleaning items in a locked place. Evidence: 1. Storage closet was unlocked and contained materials and cleaning item.

Plan of Correction: Corrected during the inspection.

Standard #: 22VAC40-191-40-D-1-B
Description: Based on record review, the center did not obtain an out of state search of central registry for staff #1. Evidence; 1. Staff #1 began employment in July, 2018. The record for Staff # 1 indicates the staff member has lived in another state in the last five years. 2. Center has a Commonwealth of Virginia CPS check on file for Staff #1 but does not have a documentation of a search conducted in the other state.

Plan of Correction: We will conduct a search and place it in the file.
